Under Ukrainian law Smiles Dental Center uses in daily practice of informed consent and agreement for provision of dental services:
• The contract for the provision of paid dental services.
• Agreement by the support work.
• The contract for orthodontic services.
• Contract for implantation.
• Informed consent for surgery.
• Informed consent for dental care of the child.
All the documents, according to the legislation of Ukraine, signed in Ukrainian, with subsequent translation into another language for foreigners.
